Abstract
The objective of this study was to determine charge cost of surgical operations for cleft lip and/or cleft palate patients perfomed in the in-patient department, Dental hospital, Prince of Songkla University during 1 October 2012 to 30 September 2015. The demographic and medical data were obtained from medical records, deformities of patients were indentified by their ICD-10 code, the major operations were identified by NHSO E-claim records and categorized each operation into subgroups by their ICD-9-CM, the payment information was obtained from the hospital revenue unit, using microsoft excel to collect and analyze data. Operation cost were collected and analyzed based on the charge cost, which was adjusted into 2016 fiscal year value. Statistical analysis included percentage, mean, and standard deviation was applied.
The results showed that, during the period of study, there were 138 cases underwent 11 types of surgical operations and the average cost for each operation were as followed, cost of cleft lip repair was 18,506.70+ 3,533.29 baht, cost of cleft palate repair was 18,068.42 ± 1,824.23 baht, cost of aveolar bone grafting was 20,365.09 + 7,499.30 baht, cost of rhinoplasty was 13,624.88 ± 1,303.57 baht, cost of maxillary orthognathic surgery was 49,469.52 ± 1,267.59 baht, cost of two jaws surgery was 68,623.68 ± 3,448.07 baht, cost of maxillary orthognathic surgery combined with DOG was 236,434.65 + 4,336.54 baht, cost of two jaws surgery combined with DOG was 276,788.41 + 9,885.57 baht, cost of DOG removal was 20,744.05 ± 10,962.18 baht, cost of plastic soft tissue revision was 12,151.42 baht and cost of tooth extraction and embedded tooth removal under GA was 22,328.25 baht. The major of composition of
the charge cost in each operation was operation and anesthetic cost, which accounted for 56.3-79.3 percent of overall cost.
The charge cost of patients who had universal coverage right were subsidized by NHSO in amount of 6,671.73 to 226,181.51 baht for each operation (which accounted for 32.97 to 81.72 percent of overall charge cost) and for the remaining cost, The Princess Mother's 100th Year Honored Fund, Faculty of Dentistry would also subdized for the patients in amount of 4,884.02 to 50,606.90 baht in each operation (which accounted for 18.28-55.21 of overall charge cost).
