Abstract

This quasi experimental research aimed to study the effect of a transition nursing program on role confidence and satisfaction among caregivers of elderly patients during the transfer from the intensive care unit. Purposive sampling was used to select caregivers of elderly patients who were admitted to the intensive care unit in a tertiary hospital. A total of 50 caregivers of elderly patients were included in this study. The first 25 caregivers of elderly patients were assigned to the control group who received usual care and the other 25 were assigned to the experimental group who received a transition nursing program twice a day, 30 to 40 minutes each time, for three consecutive days. Research instruments included a transition nursing program divided in 2 parts: (1) a series of nursing activities to promote the transition from critical care unit. (2) experimental tools included manual for caregivers' elderly, caregivers' need assessment, caregivers' level of confidence, and caregivers' skills assessment. Data collection instruments included: Demographic data of elderly patients and their caregivers, caregivers' role confidence questionnaire, and caregivers' satisfaction questionnaire. All study instruments were verified by three experts for content validity. Reliabilities of the role confidence and satisfaction questionnaire were evaluated using Cronbach's alpha coefficient. Reliability of the role confidence of elderly caregivers and the satisfaction questionnaire were .81 and .82, respectively. Data were analyzed using descriptive and inferential statistics. The results were as follows: (1) The mean score of the role confidence after receiving the transition nursing program for the experimental group was significantly higher than that of the control group (F = 32.60, p < 0.05). Comparison of the change in mean score of role confidence showed that the change in the experimental group after three consecutive days was significantly higher than that in the control group (p < .05). (2) The mean score of satisfaction after receiving the transition nursing program for the experimental group was significantly higher than that in the control group (F = 31.88, p < 05). Comparison of the change in mean score of satisfaction in the experimental group after three consecutive days was also significantly higher than that in the control group (p < .05). This study showed that giving a transition nursing program could increase the role confidence and satisfaction of caregivers of elderly patients during the transfer from the intensive care unit. Therefore, this intervention should be incorporated into standard nursing practice during such transfers from the intensive care unit.
