การศึกษาองค์ความรู้การรักษาโรคเบาหวานของหมอพื้นบ้านจังหวัดพะเยา

Abstract

Nowadays folk healer's knowledge still plays an important role in rural resident healthcare, especially treatment by herbal medicines. The present study aimed to explore the knowledge for diabetes treatment with herbal medicines by local healers in Phayao Province. Folk healers were selected by purposive sampling. The information about healers' profile, principles of diabetes, methods of treatment and prescription of drug used was collected by in-depth interview and semi-structured interview. The results found 5 male healers that had the qualification met the criteria. Their age ranges from 68-90 years old. They have had more than 10 years of diabetes treatment. The passion for being folk healer was received from their role as healer's heir. About principle of diabetes mellitus, it was originated from any illness factors that cause imbalance of the 4 body elements. In which, the body imbalance was caused by dietary behavior and genetics. The treatment process began with gathering symptoms from patient talking and then treating by specific herbal decoction. Six herbal remedies comprising 17 species were used. The most relative frequency of citation (RCF) with a value of 0.60 was found in Thunbergia laurifolia Lindl. (Acanthaceae). Based on the pharmacological properties reviewed for each herb in the prescriptions, further studies could be conducted in animal models and clinical trials to confirm the efficacy, effectiveness, and safety of those folk healers' diabetic drugs in subsequent stages.

ในปัจจุบันองค์ความรู้ภูมิปัญญาของหมอพื้นบ้าน ยังคงมีบทบาทสำคัญในการดูแลสุขภาพของประชาชนในชนบท โดยเฉพาะการรักษาโรคด้วยยาสมุนไพร การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาองค์ความรู้การรักษาโรคเบาหวานด้วยยาสมุนไพรของหมอพื้นบ้านจังหวัดพะเยา คัดเลือกหมอพื้นบ้านโดยการเลือกแบบเฉพาะเจาะจง ศึกษารวบรวมข้อมูลภูมิหลังการเป็นหมอพื้นบ้าน หลักการและแนวทางการรักษารวมทั้งตำรับยาที่ใช้รักษา โดยใช้แบบสัมภาษณ์กึ่งโครงสร้างร่วมกับการสัมภาษณ์แบบเจาะลึก จากการศึกษาพบข้อมูลหมอพื้นบ้านที่มีคุณสมบัติตรงตามเกณฑ์จำนวน 5 คน เป็นเพศชายทั้งหมด มีอายุระหว่าง 68-90 ปี มีประสบการณ์ในการรักษาโรคเบาหวานมากกว่า 10 ปี มีแรงจูงใจในการเป็นหมอพื้นบ้านจากการเป็นทายาทผู้สืบทอด แนวคิดเกี่ยวกับโรคเบาหวานเกิดจากโรคหรืออาการ เจ็บป่วยที่เกิดขึ้นกระทบกับธาตุทั้งสี่ส่งผลให้ร่างกายขาดความสมดุล เกิดขึ้นได้จากพฤติกรรมการรับประทานอาหารรวมถึงพันธุกรรม โดยกระบวนการการรักษาเริ่มจากการสอบถามอาการจากคำบอกเล่าของผู้ป่วย และทำการรักษาโดยการจ่ายยาในรูปแบบยาต้มเฉพาะราย พบการใช้ยาตำรับจำนวน 6 ตำรับ ประกอบด้วยสมุนไพรทั้งสิ้น 17 ชนิด ค่าดัชนีความแพร่หลายของการใช้ประโยชน์จากพืช (RFC) พบว่า พืชที่มีค่าดัชนีรายงานความแพร่หลายของการใช้ประโยชน์จากพืชมากที่สุดคือ รางจืด พืชในวงศ์ Acanthaceae มีค่า RFC เท่ากับ 0.60 จากข้อมูลพื้นฐานการทบทวนฤทธิ์ทางเภสัชวิทยาของสมุนไพรในตำรับแต่ละชนิด สามารถนำมาใช้ในการศึกษาเพิ่มเติมในระดับสัตว์ทดลองและศึกษาในระดับคลินิก เพื่อยืนยันประสิทธิภาพและประสิทธิผลรวมถึงความปลอดภัยของตำรับยารักษาโรคเบาหวานของหมอพื้นบ้านได้ในลำดับต่อไป
