Abstract
Abstract
This practical action research aimed to study the situation of incident reporting,
develop an incident reporting model, and study the effect of using shared values to report
incidences in Bangkok Surat Hospital. The study applied the systemic framework (Donabedian,
2005) and the action research process consisting of 4 phases: planning, action, observation and
refection (Kemmis & McTaggart, 2014). Data were collected through group discussion, semistructured interview, questionnaires, and observation. Quantitative data were analyzed using
descriptive statistics. Qualitative data were analyzed using content analysis.
Results showed that: 1) Incident reporting situation in 3 aspects were: (1)
structure: lack of continuing policy communication. and lack knowledge, understanding, and
multi-disciplinary participation among personnel. (2) process: inefficient of information
technology management and responses to incidence reports. and (3) outcome: incidences were
under report. The report was about incidences of others more than the reporter’s unit. The
attitude to the reporting was for the negative incidences. 2) The shared value model of incident
reporting consisted of 4 parts: (1) leading organizations, (2) teamwork, (3) empowerment &
errorless, and (4) value on staff or LTEV model. This model was implemented into incidence
reporting system in 3 stages: (1) detection (2) analysis and (3) learning. There were 82 % of
personnel following the guideline of incident reporting during the implementing of the LTEV
model. Leader, technology, and communication were keys of success for this model. 3) The
results of using the model found that the overall practice was at their individual level (M = 2.16,
SD = 0.51). The overall of realization score was high (M = 4.34, SD = 0.51). Awareness difference
comparison results before and after. The differe was statistically significant (t = -8.13, p < 0.05).
The number of incidence reports increased from 22 cases to 202 cases, and the department
self-reporting increased by 5%. Nursing administrators can use the results as a guideline for
creating share values in other departments. To ensure patient safety.
