รูปแบบการดูแลแบบประคับประคองเพื่อพัฒนาสุขภาพจิตและคุณภาพชีวิตของผู้ป่วยมะเร็ง ที่ได้รับรังสีรักษาในอาคารเย็นศิระ

Abstract

งานวิจัยนี้เป็นการวิจัยกึ่งทดลอง แบบกลุ่มเดียววัดก่อน-หลังการทดลอง มีวัตถุประสงค์เพื่อศึกษาสภาพการให้บริการแก่ผู้รับบริการที่อาคารเย็นศิระ พัฒนารูปแบบการดูแลแบบประคับประคองและเปรียบเทียบสุขภาพจิตและคุณภาพชีวิตของผู้ป่วยและญาติผู้ดูแลหลักก่อนและหลังได้รับการดูแลแบบประคับประคอง กลุ่มเป้าหมายได้มาโดยการเลือกแบบเจาะจง จำนวน 29 คน เป็นผู้ป่วยมะเร็งที่รับการการรักษาโดยการฉายรังสีและพักในอาคารเย็นศิระกับญาติผู้ดูแลหลัก และมีความสมัครใจเข้าร่วมการวิจัย แบ่งเป็นกลุ่มผู้ป่วย 14 คนและญาติผู้ดูแลหลัก 15 คน เครื่องมือที่ใช้ในการดำเนินการวิจัยประกอบด้วยแบบบันทึกการสัมภาษณ์ แบบสอบถามสุขภาพจิตฉบับภาษาไทยของกรมสุขภาพจิต ค่าความเชื่อมั่น 0.83 และแบบวัดคุณภาพชีวิตขององค์การอนามัยโลก (ชุดย่อ) ฉบับภาษาไทย มีค่าความเชื่อมั่น 0.81 การวิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาแบบ วิเคราะห์ข้อมูลเป็นร้อยละ ค่าเฉลี่ย (Mean) และส่วนเบี่ยงเบนมาตรฐาน (SD) และใช้ Paired-Samples t-test ในการทดสอบสมมุติฐาน ผลการวิจัยพบว่า 1. สภาพการให้บริการผู้ป่วยและญาติผู้ดูแลหลักที่พักอาคารเย็นศิระ มีสัดส่วนของเจ้าหน้าที่ที่ให้บริการไม่ครอบคลุมตามความต้องการของผู้รับบริการ 2. รูปแบบการดูแลแบบประคับประคองผู้ป่วยมะเร็งที่ได้รับรังสีรักษาในอาคารเย็นศิระ จัดแบ่งได้ 3 ระยะ ได้แก่ ระยะแรกรับ ระยะทำการรักษา และระยะก่อนกลับบ้าน 3. สุขภาพจิตของผู้ป่วยและญาติผู้ดูแลหลักอยู่ในระดับปกติ ค่าเฉลี่ยคะแนนสุขภาพจิตหลังใช้การดูแลแบบประคับประคองมีค่าลดลงอย่างไม่มีนัยสำคัญทางสถิติ ที่ระดับ .05 นั่นคือ ผู้ป่วยและญาติผู้ดูแลหลักมีสุขภาพจิตดีขึ้น ส่วนคุณภาพชีวิตของผู้ป่วยและญาติผู้ดูแลหลักโดยรวมทุกด้านคะแนนเฉลี่ยอยู่ในระดับปานกลาง This study was a quasi-experimental research using one group pretest-posttest design. The purpose of this research is to study the current conditions of service personnel at Yensira Building, develop palliative care model and compare the effect of a palliative care model performed on before and after implement this model on mental health and quality of life for radiotherapy received cancer patients and caregiver of patients at Yensira Building. Twenty-nine samples via purposive selection were divided into 14 cancer patients and 15 caregivers. The research instruments composed of case record form (CRF), general health questionnaire (GHQ) of department of mental health and short form questionnaire for quality of life (WHOQOL-THAI) with reliability of 0.83 and 0.81, respectively. Data were analyzed using descriptive statistics such as percentage, mean, and standard deviation. Hypothesis test with Paired-Samples t-test. The results are as follows: 1. The number of medical personnel and officials are insufficient to serve cancer patients and their caregivers not covered by service needs at Yensira Building. 2. Palliative Care Model in Cancer Patients with Radiotherapy in Yensira Building is divided into 3 phases : Before treatment, During treatment And before leaving home. 3. The mental health of the cancer patients and their caregivers before and after using palliative care model was normal mental health status. However, the cancer patients and their caregivers treated with the palliative care model achieved a non-significantly lower mean post-test adjustment scores than their pre-test scores and Quality of life of the cancer patients and their caregivers was moderate levels.
